Details, position and availability first
Name and contact, the position applied for, the earliest start and the days and hours available: the form opens with what the manager needs to decide whether to read on. A form that opens with a page of declarations loses the applicant before the work history.
Work history in reverse order, references on the same page
Employer, role, dates and the reason for leaving, most recent first, for as many jobs as the role warrants, and references with a name, a relationship and a way to reach them. The form should ask for what a manager will check, and nothing it will not.
Declarations and consents that your state allows, and none it forbids
A statement that the answers are true, consent to contact references, and whatever your state permits on eligibility to work; salary history, arrest records and protected characteristics are restricted or forbidden in many states. The terms say so, and the form leaves them out.

Is there a free job application form?

Can a job application form ask about criminal history?
In many states and cities, not on the application: ban-the-box laws defer the question until later in the process, and some forbid it entirely for certain roles. Whether and when you may ask is a question for your state and your adviser, and the form leaves it out by default.
