Your multiple times the rent is what the policy needs
A screening policy usually says an applicant's gross monthly income must be some multiple of the rent, and the form takes that multiple as yours and shows the income it implies for this unit. Applied the same way to every applicant, it is a rule; applied differently, it is a fair housing complaint, and the terms say so.

Income over rent is the multiple the applicant has, and rent as a share of income is the same fact the other way round, which is how a lender or a guarantor would read it. Both are shown so the comparison with the policy is arithmetic on the form rather than a judgement in the office.
The burden with other payments, and the record kept
Rent plus the other monthly payments the applicant declares, as a share of income, is the burden the household will carry, and it is the figure that separates two applicants with the same income. The paid plan keeps every application against the applicant with the figures and the decision.

What income multiple should a landlord require?
Your own, applied to every applicant the same way; the worked example uses 3 to show the arithmetic and nothing more. Some cities restrict or forbid income requirements, and fair housing law constrains screening everywhere; the rule is yours and your adviser's, and the form publishes none.
Does the form decide whether the applicant qualifies?
No. It shows the figures your policy compares and leaves the decision to you, because the decision is yours to make and to defend. It does not run a credit or background check, and it does not screen on anything but the numbers entered.
