Who and where, then what in the customer's words
Name, contact and the address the work is at open the form, then the request in the customer's own words, because the description a customer writes is the one they will hold the business to. A tick-list of services helps the office; a free-text line helps the visit.
When, and how urgent, as a date and a choice
The date the customer wants and whether it can wait are two fields, not one, and they decide the schedule and the price. A request that says urgent and next Tuesday is two requests, and the form should make the customer choose.
