This is an order form that adds itself up in the order the customer reads it. Three lines at their quantities and unit prices make the goods subtotal; sales tax at your rate applies to the goods; shipping or delivery goes on its own line untaxed; and the order total is the last and largest figure, with the units on the order beside it for the picker. Every figure is yours; the form publishes no tax rate. Free, on the page, no account; the paid plan saves every order against the customer.

Each line is a quantity times a price

An order is lines, and each line is a quantity times a unit price, worked on the form so the customer sees what each thing cost rather than one figure at the end. Three lines cover most counter orders; the paid plan carries as many as the order needs.

Tax on the goods, at your rate, shown as a line

Sales tax applies to the goods subtotal at your state's and your city's rate, and the form shows the tax as its own line so the customer sees the base and the rate rather than a total that is somehow higher. The rate is yours to enter; the form publishes none.

Shipping untaxed on its own line, then the total and the units

Shipping or delivery goes on after the tax, untaxed, which fits the states that do not tax separately stated shipping. The total is goods plus tax plus shipping, and the units on the order sit beside it so the person packing the order can count the box against the form.

Order form: common questions

Is this a free order form template?

Yes. Enter the quantities and prices on up to three lines, the shipping and your tax rate, and it works out the subtotal, the tax, the total and the units on the page with no account and no card. What tax rate should I enter?

Your own: the combined state and local rate that applies to what you sell where you sell it. The worked example uses 8% to show the arithmetic and nothing more. The form publishes no rate, because the right one depends on your location and your goods, and your state is the authority.

Can I take a deposit on the order form?

Take it against the total the form gives: a share of the order total at booking and the balance on collection or delivery. The paid plan keeps the order with what was paid against it; on the free form, note the deposit taken beside the total.
